YOU MAY BE ELIGIBLE TO PARTICIPATE IF YOU MEET THE FOLLOWING CRITERIA:

  • Are 40 years of age or older
  • Have a diagnosis of Cardiovascular Disease, High Blood Pressure and Type 2 Diabetes

Cardiovascular disease is a group of conditions that affect the heart and blood vessels. It can cause problems like heart attacks or strokes when blood flow is blocked or reduced. Things like high blood pressure, diabetes, high cholesterol, smoking, and being overweight can increase the risk.

High blood pressure means your blood is pushing too hard against your blood vessels. Over time, this can damage your heart, kidneys, and other organs.

Type 2 diabetes happens when your body cannot properly use insulin, causing your blood sugar to stay too high. High blood sugar over time can harm the heart, kidneys, nerves, and eyes.
